Acne and Acne Scars Are Two Different Problems
Most patients arrive asking for scar treatment while their acne is still active — and treating scars while new lesions are forming is how people end up spending for years without lasting improvement. Our dermatologists in Delhi always sequence it correctly: bring the active acne under control first, then resurface and remodel the scarring that remains. This is slower to promise but far faster to actually deliver.
Stage One: Controlling Active Acne
Acne is a medical condition with identifiable drivers — hormones, sebum production, follicular blockage and C. acnes bacteria — and it responds to medical management. Depending on your grade and pattern, treatment may include topical retinoids and antimicrobials, a short oral course, hormonal evaluation where PCOS is suspected, and in-clinic comedone extraction and salicylic or mandelic acid peels. We also review the products you are already using, because a surprising number of stubborn cases are being driven by the skincare itself.
Stage Two: Treating the Scars
Acne scars are not all alike, and the scar type dictates the treatment — which is why generic “scar packages” so often disappoint.
- Boxcar and rolling scars — respond best to fractional laser resurfacing and medical microneedling with radiofrequency, which remodel collagen from below.
- Ice-pick scars — narrow and deep; treated with focal TCA CROSS before any resurfacing.
- Tethered scars — released with subcision, then filled with collagen-stimulating treatment.
- Post-inflammatory pigmentation (dark marks) — not true scarring at all; cleared with pigmentation peels and PICO laser.
- Keloid and hypertrophic scars — managed with intralesional injections rather than resurfacing.
Most patients have a mix of two or three of these, so treatment is usually a combination protocol delivered over several sessions rather than a single modality.
How Long Does It Take?
Active acne typically shows meaningful improvement in 6 to 12 weeks of consistent medical treatment. Scar revision is a longer project — expect 4 to 6 sessions spaced 4 to 6 weeks apart, with collagen remodelling continuing for months after the final session. Realistically, good scar treatment improves scarring by 50 to 80 percent; any clinic promising complete erasure is overselling.
What We Will Not Do
We will not resurface skin that still has active inflammatory acne, we will not sell a laser package to someone whose pigmentation would clear with topicals, and we will not put you on long-term oral antibiotics without a plan to exit them. Honest sequencing costs you less and works better.
